Form 1098T is used as a guide in the preparation of your taxes. Its purpose is to assist you in determining your eligibility for an education tax credit.
Employer Tuition Reimbursement
This form is now available through . To access this form, an account will need to be created. Once the account has been created, you will then go to “Manage Requests” and choose Student Billing. Complete this form and submit it along with any remaining balance due to the Office of Student Billing. A $40 employer reimbursement plan fee will be added to your account. Please submit a new form each semester.
If you are an international student and are unable to create an account in Student Forms, please reach out to the Student Billing Office via email, and a form will be provided for you.
Refund Information
- Direct Deposit Cancellation Form
- If you wish to discontinue the direct deposit of financial aid refunds, please log in to My亚色影库 and inactivate your current banking information.
- You can update your banking information by logging in to your My亚色影库 account.
- Request Stop Payment of Refund Check
- To request a stop payment, please stop in our office or email us from your 亚色影库 email. Your request should indicate that you are requesting a stop payment of a refund check. Please include the amount and date the refund was processed, as well as your banner ID.
Note: By doing this, you understand that after you initiate this request, a stop payment will be placed, and if you find the refund check, you will not cash it. 亚色影库 will reissue the refund check after the original has been stopped and voided.

Note: By completing this, you are giving permission to the university to pay any outstanding balance on your student account (including non-institutional charges) for the same aid year. This will not cover charges from prior aid years.

Once your Wellness Fee Exemption Form request is approved, you will no longer be authorized to use any services that are financed with funds from the Wellness Fees. This includes use of the Center of Health and Well-Being.
